Terms & Enrollment
This is a discount plan, not dental insurance, and it cannot be combined with insurance, workers’ compensation, or promotional offers. Payment is due at the time of service, and the membership is non-refundable—even if unused. The plan auto-renews annually, with a 14-day notice required for cancellations or changes. Terms are subject to yearly updates, which will be communicated via email. Orthodontic treatment requires active membership for the full course of care. To enroll, speak with our Office Manager and complete a short form. Your membership becomes active on the first day of your sign-up month.
